Preparing for your lab test
We require a physician's order for our laboratory services. The order must be signed and dated by a physician from the state of Florida. It also must include information regarding the diagnosis and testing needed.
Please bring the following items to your test:
- A physician's order that meets all of the above criteria
- Insurance information
- Valid photo identification
